On July 1, 2006 a program called the Minnesota Family Planning Program became available to many low-income Minnesota men and women. This program provides free family planning services including yearly exams and birth control at Family Tree Clinic.

You are eligible for this program if you:

• Are between the ages of 15 and 50
• Are a MN resident
• Are a U.S. citizen or a non-citizen who is qualified to participate in federally funded programs
• Have an income at or below 200 percent of federal poverty guidelines (under $1805 gross monthly income for a single person). Your parents’ income is not considered in the application.
• Are not pregnant
• Are not enrolled in Medical Assistance, MinnesotaCare or General Assistance Medical Care.
