Minnesota Family Planning Program

On July 1, 2006 a program called the Minnesota Family Planning Program became available to many low-income Minnesota men and women. This program provides free family planning services including yearly exams and birth control at Family Tree Clinic.

You are eligible for this program if you:

To apply, download a Minnesota Family Planning application (pdf, 756kb) and bring the completed application to your appointment at Family Tree.
